Approved Course Attendance Records

Your attendance at approved courses provided by the FPA of Greater Rochester New York is reported directly to the NYS Insurance Department.  These CE credits will appear on your online license renewal form.  Confirming documentation will also be given or mailed directly to qualifying program attendees who followed proper sign-in/sign-out procedures and supplied the necessary information for state reporting.  If you believe any credits offered by the FPA of Greater Rochester NY are missing, please contact the chapter office at 585-410-1462.  You will need to provide your name and the approximate dates of the program(s) in question.  Please note that you are required to provide your insurance license number and the last for digits of your social security number when you register for events.  Failure to supply this key information will prevent us from reporting your credits to the state.  An administrative fee may be assessed if you request replacement documentation.
